The Top 3 Things Every Person with Diabetes Should Know back to the top

1) Become Informed
• Knowledge is your first step to understanding the disease and gaining control over your health.
• Knowledge can be obtained from several sources in your community…

i) Algoma Diabetes Education and Care Program (ADEC) a program of the Group Health Centre, is located at 83 Willow Avenue and offers a full range of services including adult, pediatrics, pregnancy and diabetes prevention. Individual assessments, education, care and follow up is provided to all clients through a combination of individual and group sessions. Specialty education classes are available to all clients, these include label reading, low cholesterol, stress management and foot care. These services are available by self-referral or by referral from your physician. Appointments can be made by calling 541-2670.

ii) Canadian Diabetes Association located at 134 Northern Avenue East can provide you with up to date literature and research information and can connect you to the community resources available to help you. Please visit the office or call 759-1233 for more information on the services available. (See below for further details.)

2) Take Action!
• Once you have armed yourself with knowledge of your disease and its complications, you are ready to move on to prevention. Use your new resources to seek out more information and professionals to assist you in preventing the complications of diabetes.

• A list of area resources and providers can be accessed through this web site.

• Make use of the resources available to help you. All resources including the Algoma Diabetes Education and Care Program and the Canadian Diabetes Association are available to you throughout your lifetime. Diabetes is complicated and progressive. The strategies you learn when you are first diagnosed may need to be altered as you age, your condition changes or new technology becomes available.

• Stay informed. Diabetes researchers make frequent advances that can optimize your diabetes care, extend your life and make managing your diabetes easier.

3) Maintain Control!
Once you have armed yourself with knowledge and professional support, take charge. Lead your health care team by doing all you can to improve your health and prevent diabetes complications. Your professional team is there for your support, but ultimately only you can prevent the serious complications of diabetes. Most importantly in your battle to prevent complications is to keep your blood glucose within the accepted range. Even if you feel well, having blood glucose levels that are chronically high can cause serious organ damage and circulatory problems and a low blood sugar can be just as dangerous. Every step you take to maintain optimal blood glucose level is one step towards improved health and a longer lifespan. Revisit your resources if you need to, and take charge of yourself and your diabetes.

Algoma Diabetes Education and Care Program(ADEC), a member program of the Northern Diabetes Health Network (NDHN) (www.ndhn.com) is hosted by the Group Health Centre. The program offers a full range of services including:

• Nurses, dietitians, and a pediatric social worker who are available to work with you and your physician in managing your diabetes.
• Adult, pediatrics, pregnancy and diabetes prevention.
• Individual assessment, education, care and followup is provided to all clients through a combination of individual and group sessions.
• Specialty education classes are available to all clients, these include label reading, low cholesterol, stress management and footcare.

ADEC also provides diabetes outreach to satellite communities in the Algoma region. As well, ADEC provides health promotion and diabetes awareness to service groups, schools and other health care agencies.

ADEC provides a variety of educational services for clients with diabetes in both an individual and/or group setting. Appointments can be made by individuals or by physician referral.

Camp Huronda is a part of a nation-wide network of camping services offered by the Canadian Diabetes Association since 1954 to children and youth with diabetes.

Camp Huronda lies on a beautifully wooded peninsula on Lake Waseosa near Huntsville. This superb 110-acre setting offers permanent camp facilities for children with diabetes 8-15 years old, for 4 two-week periods in July and August. Carefully planned day and evening programs provide a summer adventure of fun, activities, new skills and health care.

Staff: Dr. Kusiel Perlman from the Hospital for Sick Children and Dr. John VanderMeulen from Children’s Hospital at Chedoke MacMaster Medical Centre coordinate the staff and on-site physicians. They are both childhood diabetes specialists with experience at Camp Huronda. An experienced Certified Diabetes Educator, Diabetes Nurse and a Nursing Team ensure supervision of each camper’s insulin injections and testing procedures. A Registered Dietitian coordinates individual diabetes meal planning for the campers. Excellent meals and snacks are prepared in the modern kitchen and enjoyed by the campers and staff in the dining hall.

North Shore Tribal Council http://www.mamaweswen.ca/

The North Shore Tribal Council has four centres servicing Sault Ste. Marie and surrounding areas. They provide Registered Dietitian, Community Health Nurses, Lay Diabetes Educator, Nurse Practitioner and Physician services to each first nation.

Appointments are preferred and are necessary for physician services.

Registered Dietitian: Bev Humphries RD, CDE is available one day per week at each health care centre.

Nurse Practitioner: Rhonda Angeconeb, NP at the Indian Friendship Centre is available Tuesdays, Wednesdays and Thursdays.

Physician Services: Dr. Quon offers physician services throughout the North Shore Tribal Council district. He is at the Indian Friendship Centre Fridays 12:30-4:30, Garden River 5:00-9:00 on Wednesdays, Batchewana 5:00-8:00 on Tuesdays and is in Thessalon from 9:00-12:00 on Tuesdays.

Community Health Nurses: Maxine Lesage provides nursing services to Garden River.
Carol Hermiston provides nursing services to Batchewana.
Thessalon is currently without a community health nurse as the position is vacant.
Lay Diabetes Educator, Sherri Kitts, provides education on diabetes prevention through one-on-one counselling, presentations, workshops, and home visits.

Workplace Presentations back to the top

Education about all aspects of diabetes is important for people with diabetes wishing to minimize their risk of serious complications as well as for those wishing to minimize their risk of developing the disease.

The Canadian Diabetes Association offers free educational presentations at the workplace (or church, service club, weight loss centre, etc.). Presentation topics and length can be tailored to suit the specific needs of each group. Presentation packages have been designed by a national committee of health professionals and all facts are current and referenced. Staff or public education volunteers are available during working hours or evenings.

Topics include: diabetes facts, diabetes prevention, diabetes management, research, and information about the Canadian Diabetes Association. To schedule a presentation or become a Canadian Diabetes Association public education volunteer call 759-1233.
