Monitoring for Health Program - Canadian Diabetes Association

This program, housed out of the Canadian Diabetes Association, assists Ontario residents (with a valid health card), that are insulin dependent or have gestational diabetes with the cost of diabetic testing supplies.  There can be no other existing coverage for the supplies covered by this program (for examples, seniors would only be covered for lancets and a meter once every 5 years because their strips would be covered under the Ontario Drug Benefit Program - see following). When accessing this program individuals can receive a 75% reimbursement, up to a maximum of $820.00 for strips and lancets annually. The program will also cover 75% or $75.00 (whichever is less) for a blood glucose meter once every five years. For those who have diabetes (insulin using) and are visually impaired, 75% or $300.00 (whichever is less) will be reimbursed for a talking glucose meter once every five years. An initial claim form must be signed by a doctor to certify that you use insulin or have gestational diabetes. Call for more information or talk to your pharmacist.
 

Syringes for Seniors Program - Administered by the Assisted Devices Program (ADP) of the Ministry of Health and Long-Term Care

This Branch of the Ministry of Health offers a program called "Syringes for Seniors" which provides assistance to seniors who use insulin daily.  It assists those over the age of 65 and living in Ontario (with a valid health card) in purchasing needles for insulin injections through a $170.00 grant. Those who qualify must complete an application form annually to receive the grant. The Assisted Devices Program (ADP) will also assist those of any age in need of assistive devices (such as wheelchairs, prosthesis, canes etc.) by offering financial assistance.

The ODB program covers most of the cost of prescription drug products listed on the Ontario Drug Benefits Formulary (available on the above website, about 3,200 prescription drug products), as well as some exceptional cases. You must be an Ontario resident with a valid health card and be either over 65 years of age, a resident of a long-term care home, a resident of a home for special care, an individual receiving professional services under the Home Care program, on the Trillium Drug Plan or receiving social assistance (Ontario Works or Ontario Disability Support Program) to qualify. The ODB program also covers some diabetes testing supplies and nutritional products. A small deductable and dispensing fee may apply to seniors over a certain income level. All others pay up to $2 each time they fill a prescription.
 
PLEASE NOTE: Prescription drugs not listed on the Formulary are also considered for coverage through the Ministry's Exceptional Access Program (EAP) on a case-by-case basis following a written request from a patient's physician. The Trillium Drug program offers assistance to Ontario residents with a valid health card who have high prescription costs in relation to their net household income. Eligibility includes not having private insurance or having coverage that does not cover 100% of the prescription costs and not being eligible for drug coverage under the ODB program. To apply for Trillium, the individual must complete an application form that features detailed questions about their income. Based on that completed application, a deductible is determined and the individual must meet it before their coverage begins. Once the deductible is met, the individual will only have to pay a basic, low-cost dispensing fee. Most diabetes medications and blood/urine testing supplies are covered through the program, however needles are not. This program uses the Ontario Drug Benefits Plan for its coverage. Claimants may use this plan for any prescription medications; it is not limited to those associated with diabetes.  Call for more information or check with your pharmacist or health care provider to make sure that your prescriptions are eligible drug costs.
 
NOTE: You must have a prescription for strips in order to receive coverage. Your doctor can provide a prescription for you. The Trillium Drug Program does not cover needles and syringes.

This program provides eligible First Nations and Inuit peoples with a specified range of medically necessary, health-related goods and services when they are not covered through private insurance plans or provincial/territorial health and social programs. To be eligible, one must be identified as a resident of Canada and a registered Indian under the Indian Act, an Inuk recognized by one of the Inuit Land Claim organizations or an infant less than one year of age whose parent is an eligible recipient.

This tribunal hears appeals from those who feel that they have been unfairly assessed regarding social assistance that they receive from Ontario Works (OW) or Ontario Disability Support Program (ODSP).
     

Pump Assistance - Funded and administered by the Ministry of Health & Long-Term Care through the Assisted Devices Program (ADP)

The Assisted Devices Program (ADP) provides coverage for 100% of the cost of an Insulin Pump and Supplies for people with Type 1 diabetes. The program also provides a yearly grant of $2,400 for insulin pump supplies (4 cheques of $600 each).  This assistance is available to Ontario residents with a valid health card who have not been successful with multiple daily insulin injections.  Eligibility criteria includes: not having success managing with multiple daily injections; ability to properly use an insulin pump; ongoing commitment to blood glucose monitoring; and participation in a pump education program.
 
Diabetes Health Care provider will determine eligibility and assist in completing application.

If you are receiving assistance from either of these programs, your prescription drug products and test strips will be covered through your ODB card. If you are not using insulin, you may be able to receive a monthly amount to cover the cost of other supplies such as lancets. If you are using insulin, 75% of the cost of your lancets can be covered through the Monitoring for Health program (see above) and the other 25% will be covered by Social Assistance.
 
Those receiving social assistance may also be able to apply for the "Special Diet Allowance".  This allowance provides additional monthly amounts to help cover the cost of special diets for certain medical conditions. 
 
Medical Appointments:
If you are spending $15/month or more travelling to medical appointments, you may be able to get this money back. Ask your worker for more information.
