  • 83 Willow Ave.

There is only one set of doors accessible when entering any of these buildings. You will be asked a series of questions.

Some will include COVID-19 symptoms: Fever, new or worsening cough, shortness of breath, chills, runny nose, nasal congestion, sore throat, headaches, muscle or joint aches, nausea/vomiting/diarrhea, decrease or loss of taste or smell. Please answer all questions as accurately and clearly as possible. Your honesty and accuracy when answering the screening questions is essential to help us manage your visit safely.

Be advised that we are limiting companions in order to reduce the amount of people within our buildings.

We ask that all patients, visitors or companions wear a mask while within any of our buildings. If you are unable to bring your own mask, a mask will be provided to you at one of our screening stations when you first enter the building. Handmade masks are acceptable to wear, should you have your own.
It is important to understand the effectiveness of handmade masks and how to properly handle them. Please click here for more information.
